At Adonai’s Healthcare center, our mission is to love, care, protect and provide quality Home and Community based Services and TxHml services to the mentally challenged in our society. Our goal is to provide superior and outstanding services professionally designed to meet the level of need of these unique and precious individuals in our care. Achievable training goals are set for each individual and continuously monitored and modified to meet the set objective. Adonai’s healthcare is fully dedicated to providing professional services to individuals who are physically or mentally challenged.

 

SERVICES

 

Adonai’s healthcare center believes in excellent quality service and our well trained staff provides one on one services according to each individual need. Day Habilitation and residential training is available. This training stresses on realizing each individual’s maximum potentials thereby promoting their full participation in their communities. Our case managers have a wide variety of knowledge and experience in serving individuals with physical or mental challenges. Our direct caregivers are trained and are continuously being trained in handling all kinds of disabilities. Services are provided timely and accurately. Adonai’s healthcare turns disabilities into capabilities..

 

THE PROGRAM

 

Adonai’s healthcare center was created out of love, care and compassion for mentally challenged and disabled individuals in our community. When you enroll your loved one with Adonai’s healthcare, you become a part of the Adonai’s healthcare family and a partner in planning, implementing and monitoring goals and objectives that improve the conditions of the individual. You have access to residential and Day Habilitation of your loved one. You will participate along with our case managers, HCS representative, and the nurse in identifying strengths and areas of improvement. To ensure the protection and safety of these individuals, we do criminal background checks on all employees and we also check for abuse, neglect, and exploitation records. You have the choice of your loved one receiving services at the convenience of your home or at one of our residential facilities. The Texas Home Living (TxHml) Medicare waiver Program provides community based Services and supports to eligible individuals who are on HCS Medicare Waiver Program waiting list. The eligible individuals live in their own home or in their family homes or MRA facility. To enroll your loved one in Adonai’s healthcare center, contact your state school, HCS/TxHml or MRLA representative or contact us directly either by phone or fax 24 hours a day. You can also call us to inquire about our services and your options if you are already enrolled in HCS and want to transfer to Adonai’s . We can meet with you at your convenient location or at one of our residential facilities.

ELIGIBILITY

 

Individual is eligible for HCS/TxHml Program

 

Services if he/she:

 

 

 

• Qualifies for ICF/MRLOC 1

 

• Chooses HCS/TxHml over ICF/MR

 

• Meets financial criteria for the waiver program

 

• Is SSI recipient

 

• Lives in own home or family home

 

• Not concurrently enrolled in another waiver

 

  program

 

• His/her health/welfare can be assured with

 

  combination of waiver and non-waiver

 

  supports and services
